Prescription Refills:
Contact the clinic at the number above at least one week before you run out of your medications. Triplicate prescriptions must be requested 7 days prior to pick up.

Medical Records:
If you need a copy of your medical records, please complete a release form and we will forward it to Medical Records. You may download a form from www.utphysicians.com.

Parking:
Parking in the lot in front of the UT Physicians building is free.

To make an appointment for a clinic visit or house call or for further information, please call 713-486-1200 during business hours. Please plan to arrive 30 minutes before your first visit to complete paperwork. The initial visit and evaluation usually take at least 2 hours.
