For those Summer Hire positions working in a position identified as involving potentially hazardous activities or exposures, the Fair Labor Standards Act (FLSA) requires the individual to be at a minimum age of 18 years. Such activities include, but are not limited to:

Individuals who might request this form are the supervisor of the laboratory area where a tour or project is planned, the directing teacher for a UTHealth cooperative project with an area school, or a minor who wishes to work or study at the university. After the first section is completed, the minor should give the form to his or her parent or legal guardian for review and signature. In the case of a group project, the project organizer may wish to have the top section of the form completed before photocopying it for distribution to all minors participating in the project. This form should then be returned to the hiring supervisor of the designated project area for review.
