Student Health and counseling services are available to all UTHealth students.

Due to the current coronavirus pandemic, it may be necessary for students to disclose protected health information to the University regarding travel status or COVID-19 infection status. In order to release this information to the Student Affairs office, please print and sign thisform. You may contact Student Affairs or Student Health if you have any questions.
